Is It Good to Take a Multivitamin Every Day? What Labs Say - Med Matrix functional medicine blog

There's a bottle on your counter you've been working through for years. Maybe your mother took the same brand. You swallow one with coffee most mornings, you feel roughly the same as you did before, and somewhere in the back of your mind sits a question you've never bothered to answer: is this doing anything at all?

It's a fair question, and the honest answer is more interesting than either side of the usual argument. A daily multivitamin isn't a scam. It's also not the insurance policy the label implies. What it is depends almost entirely on what's actually low in your body, and most people taking one have never found out.

What the Large Studies Keep Finding

Researchers have followed hundreds of thousands of adults taking daily multivitamins, tracking whether they go on to develop heart disease, cancer, or cognitive decline at lower rates than people who skip them. The results have been consistently underwhelming. For generally healthy adults eating a reasonable diet, a standard multivitamin has not shown a clear effect on how long you live or what you're likely to be diagnosed with.

That finding gets reported as "multivitamins don't work," which is too blunt. A better reading is that they don't work as a general intervention for people who aren't deficient in anything. Fixing a problem you don't have produces no benefit, and that's exactly what the trials measured.

The deficiencies are the part worth paying attention to. Those are real, they're common, and they respond well to being corrected. The trials just weren't designed to find them.

The Problem With a Pill Built for the Average Person

Open any multivitamin and you'll find twenty-odd nutrients at doses calculated for a hypothetical adult. That person doesn't exist. You have your own genetics, your own gut absorption, your own medications, your own diet, and your own history of stress and illness. The odds that a fixed blend matches your particular gaps are not good.

Two things tend to go wrong at once. The nutrient you're genuinely short on shows up at a dose too small to move your levels, because the formula had to fit everything into one tablet. Meanwhile you're taking a dozen others you didn't need. Some of those pass through harmlessly. A few do not.

Iron is the clearest example. A woman with heavy periods may be running low and need real repletion. A man in his fifties taking a formula with iron in it, who has no shortage at all, is accumulating something his body has no easy way to get rid of. Same pill, opposite situations, and neither person has any idea which one they are.

The Nutrients That Actually Run Low

In practice, deficiencies cluster. A handful of nutrients account for most of what we find when we run labs on new patients, and they're rarely the ones people are worried about.

  • Vitamin D. The most common shortfall we see, and it isn't close. Maine winters are long, we're indoors, and sunscreen and skin tone both change how much you make. Low vitamin D shows up as fatigue, low mood, aching muscles, and getting sick more often than you should.
  • B12. Absorption drops with age, with long-term acid-reducing medication, and with metformin. Low B12 can look like exhaustion, brain fog, numbness or tingling in the hands and feet, and it gets mistaken for stress or aging constantly.
  • Iron and ferritin. Ferritin is your stored iron, and it can be scraping bottom while a standard blood count still reads normal. That's why "your labs were fine" and "I'm exhausted and my hair is thinning" coexist so often.
  • Magnesium. Involved in hundreds of reactions, depleted by chronic stress and alcohol, and poorly captured by a standard serum test. Muscle cramps, restless sleep, and constipation are the usual complaints.
  • Folate, zinc, omega-3s. Less universal, but they turn up, and each one has a symptom picture that looks like something else.

None of these are exotic. All of them are testable. And a typical multivitamin contains a token amount of each, which is enough to say the word on the label and not enough to correct a real deficit.

When a Daily Multivitamin Does Earn Its Place

Plenty of people should take one. The blanket dismissal is as lazy as the blanket recommendation.

A multivitamin makes sense if you eat a genuinely restricted diet, whether by choice or necessity. It makes sense after bariatric surgery, or with a condition like celiac or inflammatory bowel disease that interferes with absorption. Pregnancy and the months before it have their own requirements, and prenatal formulas exist for good reason. Older adults who eat less overall and absorb less of what they eat often benefit. So do people on medications that deplete specific nutrients.

Notice what those situations share. Each one is a reason to expect a shortfall. That's different from taking a multivitamin because it seems like a sensible thing to do.

Testing Beats Guessing, and It Isn't Close

Here is the part that frustrates us. Someone spends a few hundred dollars a year on supplements, takes them faithfully, and has no idea whether any of it is working, because nobody ever measured anything. They're treating a problem they've never confirmed they have, with a dose nobody chose for them, and evaluating the results by how they feel on a Tuesday.

A blood panel ends the guessing in an afternoon. Vitamin D, B12, ferritin, folate, a full thyroid picture, inflammatory markers, metabolic markers. Low ferritin plus low thyroid function plus low vitamin D tells a story that low ferritin alone does not.

What we find changes the plan. Sometimes it means a targeted, properly dosed nutrient for a few months, then a retest to confirm it moved. Sometimes it means the fatigue was never nutritional and the multivitamin was never going to touch it, which is worth knowing before another year goes by. Occasionally we find someone whose diet is genuinely covering everything, and the most useful thing we can say is to stop buying the bottle.

Is it bad to take a multivitamin every day?

For most healthy adults, a standard multivitamin at normal doses is safe to take daily. The concerns are narrower than the internet suggests: iron in people who don't need it, and fat-soluble vitamins like A and E building up at high doses over long periods. If you're taking a multivitamin plus several individual supplements, the stacking is worth reviewing with a provider, since that's where doses quietly add up.

Should I take a multivitamin if my blood work was normal?

It depends on which markers were run. A standard panel from an annual physical often doesn't include vitamin D, B12, or ferritin, so "normal" may simply mean nobody looked. Ask which nutrients were tested. If the answer is none of them, your labs haven't answered the question.

Can a multivitamin fix my fatigue?

Sometimes, when the fatigue traces to a deficiency the multivitamin happens to contain at a high enough dose. More often it doesn't, because the dose is too low or the cause was thyroid or adrenal, hormonal, or metabolic rather than nutritional. If you've been taking one for months and still wake up tired, that's information. It's pointing you toward testing, not toward a different brand.

What's the difference between a multivitamin and targeted supplementation?

A multivitamin gives small amounts of many nutrients to everyone. Targeted supplementation gives a meaningful dose of a specific nutrient to someone who has been shown to be low in it, then rechecks to confirm the level actually moved. The second approach costs less over a year in most cases, because you buy fewer things and you stop when the problem is solved.

Stop Guessing About What Your Body Needs
